Please provide the following information to help us to properly acknowledge tribute/honorary or memorial gifts donated to our agency:
        1.)  The name of the person, event, occasion, or holiday that you would like to remember
        2.)  The name and address of the person to receive notification of the tribute/honorary or  memorial gift
        3.)  The name and address of the person giving in order to send a letter acknowledging their gift.
