Group Volunteer Application
A Group Volunteer Application is for a recognized group, organization, or a group of people (e.g a church group, a civic organization, a fraternity, a sorority, etc.) who would like to volunteer at our agency.
This application consists of a two-page form, a group volunteer profile page, a criminal background check form and a central registry search form as required by the South Carolina Department of Social Services.
The two-page form is to be completed by the leader(s) of the prospective volunteer group. The criminal background check form, the central registry search form and the group volunteer profile page are to be filled out by all members including the leader(s) of the prospective volunteer group that include a photocopy of each member’s driver’s license or some type of a photo identification card.
Along with the required forms, prospective volunteers are kindly asked to pay an $16 fee for his/her criminal background check and central registry search to be done by the South Carolina Department of Social Services. Please send the completed background check and central registry forms, along with payment, to our facility: P.O. Box 503 – Aiken, SC 29802.

Or, you may call our agency to schedule a day and time to bring your volunteer application. After we receive your group’s completed volunteer application, it will take 4-6 weeks to process the application.
